Your Day with Us

Your day with us will actually begin at least two weeks before your appointment. We will ask that you complete a rather extensive health history and lifestyle assessment. This assessment can be mailed to you, or you may complete it online. Additionally, we will arrange to have your blood drawn (in your home or office) for extensive laboratory analyses. This lead-time will assure that we are fully prepared for your individualized visit.

On the day of your appointment you will arrive at 9:00, dressed in exercise clothes and shoes. During the course of your day, you will receive your CHART (Comprehensive Health Assessment, Recommendations, and Treatment), which includes:

A one-on-one review of your health history, lifestyle assessment, and laboratory results

A physical examination (please note that women will need to have had a recent mammogram and pelvic exam)

Our goal is for you to leave at the end of the day with a comprehensive plan that you can easily follow to begin the process of improving your health. As we are acutely aware that such change does not occur overnight, we will continue to work with you on an ongoing basis – fine-tuning your plan to help you achieve success (through membership in our Monitoring and Mentoring Program).
