Your CHART day with us is just the first step in your goal of achieving optimal health. Although you will leave at the end of your CHART day with a comprehensive plan and detailed instructions, you will require regular follow-up to be successful in charting a new course for your health. We will continue to work with you through our Monitoring and Mentoring Program. This program has been designed to support and encourage you as you build on your day of evaluation and begin to make the lifestyle changes that are necessary to achieve a healthier life.

There are three different membership levels. Your doctors will prescribe which level you will need, and the decision will be based on several factors. These factors include what, if any, hormones you will be taking; your current exercise and nutrition habits; psychological issues (such as stress level); and your current medical condition. The three levels are:

	Level I: This level requires the least intensive monitoring by our staff, and is designed for individuals who have an established exercise program, have no serious medical or psychological conditions which require close monitoring, and who are not prescribed any hormones. The cost for this level is $300 every six months.
	Level II: This level requires a moderate level of ongoing monitoring. Most clients placed on hormone modulation will be placed in this level. Hormone modulation requires regular follow-up visits to monitor improvement and to adjust doses to physiologic levels in order to maximize benefits while minimizing any adverse effects. The cost for this level is $540 every six months. Clients in Level II will receive a 5% discount on all Optimal Health Institute supplements.
	Level III: This level requires the most intensive monitoring. The cost for Level III is $780 every six months, and clients in this level will receive a 10% discount on all Optimal Health Institute supplements.

Additional membership benefits include:

	We pass along our (wholesale) costs for the hormones.
	Up to four follow-up office visits per year
	A yearly follow-up neurocognitive assessment
	A yearly microdermabrasion treatment
	You will have easy access to our doctors. We believe that you should be able to talk to our doctors when you need to, without having to go through unnecessary barriers. More detailed or involved telephone consultations are scheduled at mutual convenience.
	We will place your orders for prescriptions and supplements for you, and have them shipped directly to your door.

In order to participate in the program, you will need to maintain membership at the prescribed level. Membership maintains your access to our doctors’ expertise. Your physician will not continue to prescribe or manage your hormone modulation if you do not maintain membership. Please keep in mind that the cost of the labs will be in addition to the cost for membership, and will be based upon your individual program.
